Implantable left ventricular assist systems (LVASs) have recently given encouraging results in advanced heart failure (AHF). Controlled comparative trials of LVASs vs. medical therapy could confirm these results, but there are special considerations for such studies [1]. AHF patients and their physicians are reluctant to accept randomisation because, for many (younger) patients who are clearly going to die within days, they are reluctant to refuse
