Treatment of heart failure with normal ejection fraction in patients with advanced chronic heart failure

In their excellent position statement on advanced chronic heart failure (ACHF) Metra et al. [1] include some cases of heart failure with normal ejection fraction in their definition of ACHF. Although we fully agree with this inclusion, in the section on the treatment of ACHF the authors frequently seem to forget that these patients have been included in the definition. When referring to medical treatment or to the use of implantable cardioverter defibrillators the authors make statements that are only valid for patients with left ventricular systolic dysfunction. Unfortunately, only the section on aldosterone antagonists includes a statement about left ventricular systolic dysfunction. In order to avoid a confusing message, we think this point should be clarified.
